My Mission
My mission is to provide experienced care management to my clients and their family so that they may age with respect and dignity. I seek to be an asset in their life to help make day to day living stable and safe. I fully understand how hard it can be for families to navigate changes as they occur. I am able to offer guidance and assistance to help with the stress of managing these situations.
Who I Am
I have worked in the home care and care management field for over 10 years. I originally started in an administrative role learning the industry. After receiving my RN license, I began to pursue my real passion which is geriatric care management. I have experience in geriatric wellness nursing and for the last 6 years I have honed my skills to be able to provide the best care possible.
My Services
I offer a wide range of care management services to meet the needs of the client. My services include medication management, home care oversight, home safety assessment, help obtaining home care, scheduling of medical appointments, medical and legal interface, assistance with placement and much more.
